NSG 623 Chronic Disease Management of Adults and Older Adults


3 credits

This course utilizes a systems-based approach to the chronic disease management of adults and older adults in primary care.  Emphasis is placed on the diagnosis and management of common chronic health problems using current treatment guidelines and evidence-based practices, including pharmacological and non-pharmacological treatments.  Attention will be given to providing patient-centered care in collaboration with interprofessional teams and addressing social determinants of health specific to those with chronic illnesses.  The stabilization of chronic disease exacerbations in primary care settings is also covered.  Additionally, students’ preparedness to enter the clinical setting will be evaluated and hands-on skills essential for primary care will be taught.  

Prerequisite(s): NSG 540 , NSG 541 , and NSG 542
